The Afya Uzazi Program implements a quality improvement (QI) initiative that aims to improve the quality of health services through work improvement teams (WITs). In 2018, Afya Uzazi conducted a rapid project assessment and established that only 40 % of the maternity files had an accurately completed partograph and nursing notes (Cardex). The documentation lacked clarity, specificity and wasn’t complete, which contributed to ineffective continuum of care and poor quality care to the clients. Staff reluctance and inadequate knowledge, coupled by poor attitudes were identified as some of the major drivers of poor documentation.
